How to Gift this item / send to different address?

To send to another address, simply put their name and address in the contact information on this sales form. The only billing details we need are the credit card numbers. 

The email that you place on this form will receive the receipt and order confirmation. 

We do not have gift messaging or a "To/From" form at this time.
